The RN Case Manager is a position through the Comprehensive School Health Services (CSHS) grant that directly supports CPS high-risk and medically fragile students. The Case Manager will be part of an interdisciplinary approach to student health to address school-based health prevention and promotion programs (i.e., substance abuse, oral health, nutrition etc.) as well as providing support for students with chronic health conditions (i.e., asthma, diabetes etc.), addressing health disparities and racial inequities (i.e. poverty, homelessness, access to care, English language learners etc.) in order to improve student attendance and academic achievement.
In collaboration with other members of the Student Services team, the RN Case Manager is responsible for assessing, planning, delivering, and evaluating care to students and their families.
Essential Functions:
-Conducts comprehensive assessment of students' needs in health, attendance, and academic performance
-Collaborates with the nursing team to complete state mandated screenings including vision & hearing, BMI, scoliosis and SBIRT
-Collaborates with principal, school counselors and nurses to identify students for initial referrals and ongoing coordination of student services
-Completes monthly and annual state data reports for disposition of students and staff utilizing Health Services, tracking percentages of screening referrals, chronic absenteeism, obesity, ELL, homelessness, bullying, LGBTQ, poverty, substance abuse, mental health issues, etc.
-Collaborates with students/families in developing care plan and strategies for addressing identified student needs
-Refers students/families to care providers, community resources/agencies as needed
-Demonstrates cultural competency in all interactions with student/family and the community at large
-Communicates with team members weekly on student/family status
-Organizes and facilitates implementation of school dental program
-Continuous quality improvement and evaluation of programs and interventions
Qualifications:
-Current license as a Registered Nurse in Massachusetts
-Current DESE certification
-Experience in case management, home health, school or hospital nursing
-Valid MA drivers license
-Ability to work with diverse cultures and languages
-Ability to function independently and as a team member
-At least 3 years relevant experience preferred
-Bachelor degree preferred
